The course shows you how to work with people in a person-centred way to facilitate personalised care and support planning. It also shows you how to enable people in receipt of care and support services to become active participants in planning their own care.

Module 1
By the end of this module, you will be able to:

  • Understand that personalised care and support planning results in a different relationship between the service user and the practitioner
  • Explain why personalised care and support planning is important
  • Discuss some of the differences between personalised care and support planning and historical/traditional ways of planning

Module 2
By the end of this module, you will be able to:

  • Develop (in your own mind) a clear understanding of the personalised care and support planning process
  • Explain how personalised care and support planning works
  • Discuss the main focus of conversation when gathering information for personalised care and support planning

Module 3
By the end of this module, you will be able to:

  • Produce personalised care and support plans using a person-centred approach
  • Use a selection of person-centred thinking tools when supporting people to plan their care and support

Module 4
By the end of this module, you will be able to:

  • Apply the person-centred approach to care and support planning for people with complex/long-term conditions
  • Discuss how to develop suitable outcomes for people through personalised care and support planning
